The most likely cause is that the user's password was incorrect. Other root causes may be that I2B2's PM cell does not recognize the user, that SHRINE is misconfigured, or that the PM cell is malfunctioning.
If you are a user, supply all details of this error to your local site admin.
If you are a site admin, check shrine.conf, ensure that the URL for the PM cell is correct, ensure that i2b2 is responsive and reachable as configured.
